Fistulous connection between the left anterior descending coronary artery and pulmonary artery.

چکیده
DESCRIPTION A 65-year-old woman who had presented with exertional breathlessness was found to have severe mitral regurgitation secondary to mitral valve prolapse, on transthoracic echocardiography. She was admitted for diagnostic coronary angiography, prior to mitral-valve surgery. The major epicardial coronary arterieswere demonstrated to be free of disease, and left ventriculography confirmed severe mitral regurgitation. However, an anomalous arterial connection was seen to arise from either the left main coronary artery (LM) or the proximal left anterior descending coronary artery (LAD). In the left anterior oblique cranial projection, the anomalous vessel had the striking appearance of a ‘Valentine Heart’ (figure 1). Subsequently, at the time of mitral valve surgery, the anomalous vessel was shown to be a fistulous connection between the LAD and the pulmonary artery (PA) and was ligated during the procedure. Coronary arteriovenous fistulae are rare malformations, accounting for 0.2–0.4% of congenital cardiac anomalies. They may be associated with a continuous murmur or may be an incidental finding on coronary angiography. They may result in a ‘steal’ phenomenon causing ischaemic symptoms in the absence of occlusive coronary disease and in severe cases can lead to heart failure. Fistulous connections between the coronary arteries and the PA are well described in the literature. 4 Successful surgical repair of coronary-pulmonary fistulae is also well documented. 6 Due to the potential for serious clinical sequelae, some authors have advocated that all coronary arteriovenous fistulae should be corrected surgically or by transcatheter closure. This can easily be achieved at the time of surgery for concomitant cardiac conditions, although the optimal management strategy for isolated coronary fistulae remains unclear.

Association Between Left Anterior Descending Artery Length with Coronary Artery Dominance: An Angiographic Study
Introduction: Although left anterior descending (LAD) artery supplies a large portion of the myocardium, the amount of blood supply provided by LAD is depended on the length of the LAD. The aim of the current study was to evaluate the association between coronary artery dominance and LAD anatomic types in patients with normal epicardial coronary arteries. Methods:...
متن کاملFunctional assessment of a left coronary-pulmonary artery fistula by coronary flow reserve
We report a 71-year-old man who presented with atypical chest pain. Coronary angiography did not reveal left main or proximal left anterior descending coronary artery stenosis, but a fistulous communication with a stronger tube-like fistula was present originating from the proximal left anterior descending coronary artery and emptying into the main pulmonary artery. Fractional flow reserve and ...
متن کاملA Rare Case of Double Myocardial Bridges of the Left Anterior Descending Artery
Myocardial bridge is a benign congenital anomaly in which a segment of major epicardial coronary arteries descends into the myocardium for a variable distance. Myocardial bridge is most often single and located in the middle part of the left anterior descending artery. The presence of more than one myocardial bridge in coronary arteries is very rare. We present a patient with more than one myoc...
متن کاملEarly Clinical Outcomes and Mortality of Coronary Endarterectomy in the Left Anterior Descending Artery: A Single-Center Experience
Introduction: Although coronary endarterectomy is not an ideal procedure, it is the only available option for cardiac surgeons in some patients with diffuse coronary artery disease. Based on the majority of conducted studies, the results of coronary endarterectomy (including survival, graft patency, and recurrence of the symptoms of myocardial ischemia) are less prevalent than standard coronary...
متن کاملGiant Coronary Artery Aneurysm With Fistula to the Pulmonary Artery Complicated by Frequent Ventricular Premature Contractions
Giant coronary artery aneurysm with a fistula is a rare condition. The presence of a giant aneurysm imposes considerable health risks. We report a case of a 67-year-old woman who presented with frequent ventricular premature contractions caused by a giant coronary aneurysm arising from a branch of the left anterior descending coronary artery that had a fistulous connection to the pulmonary arte...
